Abstract

BACKGROUND:

Treatment of developmental dysplasia of the hip (DDH) using the Pavlik harness has been a widely used method in patients between 0 and 6 months of age for many years. However, the factors influencing the success rate of this treatment modality have still not exactly been determined as a result of the limited number of clinical studies with higher level of evidence.

QUESTIONS/PURPOSES:

We, therefore, asked whether (1) patient-related variables such as age, gender, and laterality; coexisting risk factors including family history, breech presentation, intrauterine packing, first-born girl, oligohydroamnios, and swaddling; and (2) the severity of hip dysplasia, defined by ultrasonography, are associated with differences in the success rate of Pavlik harness treatment in infants with DDH.

CONCLUSIONS:

We conclude that Pavlik harness treatment is less effective in children at and over the age of 4 months at the time the harness is first applied as well as in hips with complete dislocations and hips with severely deficient acetabular bony roofs. In such older patients and worse hip types, the use of initial Pavlik harness treatment needs to be revisited. Future studies, comparing the outcomes of the Pavlik harness treatment and other types of interventions in such patients and hip types, are needed.

Resumen
ANTECEDENTES:

El tratamiento de la displasia del desarrollo de la cadera (DDC) con el arnés de Pavlik ha sido un método ampliamente utilizado en pacientes entre 0 y 6 meses de edad durante muchos años. Sin embargo, los factores que influyen en la tasa de éxito de esta modalidad de tratamiento aún no han sido determinados exactamente como resultado de la cantidad limitada de estudios clínicos con más alto nivel de evidencia.

PREGUNTAS / propósitos:

Nosotros, por lo tanto, preguntamos si (1) las variables relacionadas con el paciente, como la edad, el sexo y lateralidad; factores de riesgo coexistentes incluyendo antecedentes familiares, presentación de nalgas, embalaje intrauterina, primogénito chica, oligohydroamnios y pañales; y (2) la gravedad de la displasia de cadera, definida por ecografía, se asocian con diferencias en la tasa de éxito del tratamiento arnés de Pavlik en lactantes con DDH.

CONCLUSIONES:

Llegamos a la conclusión de que el tratamiento arnés de Pavlik es menos eficaz en los niños en y sobre la edad de 4 meses en el momento del arnés se aplicó por primera vez, así como en las caderas con luxaciones completas y caderas con muy deficientes techos óseos acetabulares. En estos pacientes de mayor edad y tipos de cadera es peor, el uso del tratamiento inicial arnés de Pavlik necesita ser revisada. Los estudios futuros, comparando los resultados del tratamiento arnés de Pavlik y otros tipos de intervenciones en este tipo de pacientes y tipos de cadera, se necesitan.
