Abstract

BACKGROUND::

Multiple hereditary exostoses (MHE) affect the distal forearm in 30% to 60% of patients with this condition. Many surgical interventions have been described, but treatment is controversial. We present the radiographic results of low risk, minimally invasive hemiepiphyseal stapling of the distal radius to correct the joint angulation caused by MHE through growth modulation.

METHODS::

A retrospective review was performed on 18 forearms affected by MHE that underwent radial hemiepiphyseal stapling with follow-up of ≥2 years. The radial articular angle, carpal slip, ulnar tilt, lunate subsidence, and metaphyseal epiphyseal angle were measured on preoperative and final postoperative radiographs and compared.

RESULTS::

Statistically significant (P<0.05) improvements were found in 4 of 5 measurements, including the radial articular angle, carpal slip, ulnar tilt, and metaphyseal epiphyseal angle. No statistical difference was found in lunate subsidence (P=0.84).

CONCLUSION::

Hemiepiphyseal stapling of the radial side of the distal radius is a simple and effective method for correcting the radiographic deformity of the distal radius and may be an attractive option for the treatment of the distal radial deformity that is characteristic of the disease.

 ANTECEDENTES :Exostosis hereditaria múltiple (MHE) afectan el antebrazo distal en 30% a 60% de los pacientes con esta condición. Muchas intervenciones quirúrgicas se han descrito, pero el tratamiento es controversial. Presentamos los resultados radiográficos de bajo riesgo, grapado hemiepiphyseal mínimamente invasivo de la extremidad distal del radio para corregir la angulación de las articulaciones causado por MHE través de la modulación del crecimiento.MÉTODOS ::Una revisión retrospectiva se realizó en 18 antebrazos afectados por MHE sometidos hemiepiphyseal radial engrapado con seguimiento de ≥2 años. El ángulo articular radial, deslizamiento carpiano, inclinación cubital, hundimiento semilunar, y el ángulo de las epífisis metafisaria se midieron en las radiografías postoperatorias preoperatorios y final y se compara.RESULTADOS ::Estadísticamente significativa (p <0,05) las mejoras fueron encontrados en 4 de 5 mediciones, incluyendo el ángulo articular radial, deslizamiento carpiano, inclinación cubital, y el ángulo de las epífisis metafisaria. No se encontró diferencia estadística en el hundimiento semilunar (P = 0,84). 

CONCLUSIÓN :El engrapado de la hemiepifisiodesis del lado radial del radio distal es un método sencillo y eficaz para corregir la deformidad radiográfica del radio distal y puede ser una opción atractiva para el tratamiento de la deformidad radial distal que es característico de la enfermedad.
